Brief Description about the Project:

The SOHAM Cohort study has been conceptualised to assess the “Healthy Ageing Tool –India (HAT-I)” which will be developed and validated. This tool should be able to distinguish between those with a “healthy Ageing phenotype” and those without. A large cohort of volunteers (> 40 years) will be recruited. The validated HAT-I tool will be applied to them, and they will be categorised as being at different levels of “Healthy Ageing” based on their score. This cohort will then be followed up longitudinally for 5 years, with semi-annual and annual contacts to identify the occurrence of adverse outcomes in different domains. This will be used to measure the strength of association between the baseline HAT-I score and the occurrence of adverse outcomes.

Experience: Preference for candidates with experience in qualitative research, coding and maintaining the databases.

Salary: Rs. 30,000-/-pm                       

Preferred Qualifications: Bachelor's or master’s degree in social work

Roles & Responsibilities:

  • He/she should provide counselling, information and support as appropriate to patients and their families.
  • He/she should conduct psychosocial assessments where social issues are a factor in illness management and advise the multidisciplinary team on the outcome.
  • Should be able to formulate discharge care plans for patients together with the multidisciplinary team.
  • Should assess patients’ support system in order to ensure that, patients have an appropriate level of support for their needs in their families and community.
  • He/she should ensure the necessary liaison and cooperation with medical, nursing and other disciplines in the hospital and in consultation with them to arrange for case conferences in appropriate cases.
  • He/she is responsible in keeping accurate records of social work intervention.
